| KanCare, Kansas' Medicaid program, is using the medical home model to target high-risk patients diagnosed with Serious Mental Illness (SMI). Behavioral Health Homes provide six core services: comprehensive care management; care coordination and health promotion; comprehensive transitional care (including appropriate follow-up from inpatient to other settings); individual and family support; referral to community and social support services, if relevant; and the use of health information technology to link services. An expansion of the model to serve people with chronic conditions like asthma or diabetes has been delayed. | |
