The Homeless-Oriented Primary Care Clinic was a population-tailored primary care clinic for homeless veterans. The program aimed to engage veterans in clinical care early on in their homelessness (before complications arise) by using Patient-Aligned Care Teams to provide wrap-around and comprehensive medical, mental health, case management and social services in a single setting (see: Homeless Patient-Aligned Care Teams, Multistate). |
