Alaska Medicaid Coordinated Care Initiative

Alaska

Alaska was one of six states selected by the National Governor's Association in 2013 for a "Policy Academy" focusing on improving care for "super-utilizers" in Medicaid. In 2014 the state's Medicaid program began a coordinated care initiative that provides one-on-one case management to high-need patients that includes care coordination, scheduling appointments, addressing barriers to care and referrals to specialists and social service agencies. Care-management services are provided by MedExpert and Qualis. Participation in the program is voluntary.
